A patient journey crosses forms, identity checks, payment, scheduling, clinician review, messaging, and sometimes external fulfillment. Each handoff needs an owner, timestamp, retry rule, patient-visible state, and escalation route.

Keep privacy scope visible
Do not copy clinical detail into analytics, ordinary support tickets, or retry queues merely because the integration accepts free text. Use the minimum data required for the next decision and document retention at each boundary.
